Blockchain-Based System Secures Regulatory Milestone in Pharmaceutical Development

Japanese digital healthcare company SUSMED Inc. (Tokyo: 4263) has announced a major milestone for blockchain adoption in the pharmaceutical industry after its proprietary clinical trial platform, SUSMED SourceDataSync (SUSMED SDS), contributed to securing marketing authorization for a pharmaceutical product.

The approval represents the world’s first pharmaceutical marketing authorization based on the results of an industry-sponsored clinical trial that utilized blockchain technology. The development highlights the potential of blockchain-based systems to improve clinical trial management, strengthen data integrity, and reduce the operational costs associated with pharmaceutical research.

SUSMED focuses on digital healthcare research and development, including digital therapeutics for conditions such as insomnia. The company also develops technology to support clinical trials, backed by intellectual property covering digital therapeutics and blockchain applications in healthcare.

Rising Drug Development Costs Create Industry Challenges

Global pharmaceutical research and development expenditure has increased by approximately 8.5% annually over the past decade. The growing complexity of clinical trial protocols and expanding research requirements have contributed to higher development costs.

Japan faces additional operational challenges because of labor-intensive procedures and extensive compliance practices associated with its Good Clinical Practice (GCP) regulations. These requirements have contributed to a significant productivity gap between Japan and the United States. The number of clinical trial sites managed by an individual Clinical Research Associate in Japan is reportedly only one-third of the corresponding figure in the United States.

Higher development expenses and inefficient clinical trial procedures can delay the availability of new medicines, contributing to drug lag and drug loss. These challenges affect patients who may not have timely access to treatments already available in other countries.

The updated international clinical trial guideline, ICH-E6(R3), has emphasized a Quality by Design (QbD) approach. It encourages pharmaceutical sponsors to eliminate unnecessary complexity, reduce administrative burdens on investigators and participants, and establish appropriate quality management throughout the clinical trial process.

SUSMED SDS Reduces Clinical Trial Workload

SUSMED developed its blockchain-powered clinical trial management system to address these operational challenges. The platform securely synchronizes medical data collected at healthcare institutions with corresponding Case Report Form (CRF) information.

This approach reduces manual data entry requirements at medical institutions while limiting the need for certain regulatory monitoring activities. Blockchain technology also provides a mechanism for verifying the integrity and consistency of clinical trial information.

The company initially demonstrated the technology through Japan’s Regulatory Sandbox program. Results from the demonstration were published in a peer-reviewed international medical journal in June 2020.

Following the verification process, SUSMED submitted an application under the System of Inquiry on Regulatory Gray-Zone Areas established through the Industrial Competitiveness Enhancement Act. On December 4, 2020, Japan’s Ministry of Health, Labour and Welfare issued an official notification confirming that blockchain-based technology could replace certain monitoring activities while remaining compliant with the country’s GCP regulations.

A subsequent comparative assessment conducted under a research initiative supported by the Japan Agency for Medical Research and Development (AMED) demonstrated measurable operational improvements.

The evaluation found that the SUSMED SDS workflow reduced clinical trial monitoring workloads by 45.9%, workloads at medical institutions by 32.0%, and data queries related to verification and correction by 46.7%.

These results indicate that blockchain-based clinical trial management can substantially improve productivity while supporting the quality management objectives established under international QbD principles.

Global Standards Recognize Blockchain’s Potential

Industry recognition of blockchain technology in pharmaceutical research has also expanded. The second edition of GAMP 5, published by the International Society for Pharmaceutical Engineering in 2022, identified blockchain as a potential solution for strengthening data integrity in regulated environments.

This recognition reflects growing interest in applying distributed ledger technology across clinical research and pharmaceutical development.

SUSMED Plans Broader Clinical Trial Adoption

SUSMED President and CEO Taro Ueno said the achievement represented an important step toward addressing drug loss and improving access to treatment options in Japan. He highlighted the company’s earlier regulatory demonstrations in 2020 and the subsequent approval as evidence that blockchain technology could support efficient compliance without compromising clinical trial standards.

Government representatives also welcomed the development, describing it as an important example of regulatory innovation and the practical application of emerging technologies to healthcare challenges.

SUSMED plans to expand the implementation of its blockchain-based platform across a broader range of pharmaceutical clinical trials, with the objective of improving development efficiency, reducing operational expenses and strengthening research quality.

The company also expects its technology to contribute to a more sustainable clinical trial ecosystem, potentially supporting better access to innovative treatments while helping manage long-term healthcare and social security expenditure.
