Argentina Uses IOTA Blockchain to Secure Organ Transplants

INCUCAI Introduces Decentralized Digital Identity System

Argentina’s national medical transplant authority, INCUCAI, has introduced decentralized digital identities and blockchain-based document verification across its organ and tissue transplant network, marking a new use of distributed ledger technology in the country’s public healthcare infrastructure.

The system was developed by Extrimian and is designed to provide participating agencies with verifiable digital identities registered on the IOTA Mainnet. Under the framework, INCUCAI will continue to serve as the central governing authority, retaining responsibility for granting, suspending, and restoring the accreditation of agencies involved in the transplant process.

The initiative is intended to strengthen the integrity, traceability, and legal reliability of critical transplant procedures while protecting sensitive patient information.

The technology uses IOTA Identity, which provides decentralized identifiers and verifiable credentials based on standards established by the World Wide Web Consortium. The framework supports digital identities for individuals, organizations, and devices and combines self-sovereign identity technology with verifiable credentials and public ledger-based cryptographic proofs.

IOTA introduced the identity framework in an alpha release last year. Its design allows credential information to remain outside the blockchain while cryptographic evidence is recorded on the public ledger. This structure is intended to enable the verification of documents and identities without placing confidential information directly on the decentralized network.

Blockchain Proofs Without On-Chain Medical Records

INCUCAI has also linked its decentralized identifier to its official government domain. The connection is designed to allow users to verify that the digital identity displayed through IOTA is associated with Argentina’s national transplant authority.

According to Daniela Hansen Krogh, INCUCAI’s director of Information Technology and Information Systems, the integration of decentralized identity and cryptographic verification is expected to improve the security and traceability of key procedures while preserving the confidentiality of sensitive medical data.

Authorized participants will be able to electronically sign documents and create a cryptographic fingerprint, known as a hash, that is recorded on IOTA. The hash can subsequently be used to determine whether a document has remained unchanged since it was signed and notarized.

Patient records and medical documents will remain off-chain, while IOTA will store cryptographic proofs that allow authorized parties to verify document integrity without exposing confidential health information.

The blockchain system can demonstrate that a document has not been modified after notarization. However, the technology cannot determine whether the medical information entered into a document was accurate at the time of submission. Responsibility for the accuracy of medical information will continue to rest with healthcare professionals, accredited agencies, and the procedures governing the transplant system.

Digital Wallet Supports Authorized Participants

The initiative includes IncucaiID, a digital wallet intended for patients, healthcare professionals, and other authorized participants. The wallet will enable users to receive verifiable credentials, establish their digital identity, and electronically sign documents associated with organ donation and transplant procedures.

IOTA’s Gas Station system will cover the network costs related to recording cryptographic proofs. As a result, users will not be required to acquire IOTA tokens or manage transaction fees when using the service.

The fee-sponsored infrastructure is designed to allow healthcare participants to use blockchain-based verification tools without handling digital tokens or paying network transaction costs.

Dominik Schiener, co-founder and chairman of the IOTA Foundation, said the deployment demonstrated how open blockchain technology could support critical public services at a national scale. He indicated that the infrastructure was already being applied to processes involving decisions with major consequences for patients.

The operational rollout began in June and is expected to proceed in stages. The first phase focuses on registration for heart and lung transplant waiting lists. The system will later be extended to additional transplant waiting lists and other processes within SINTRA, Argentina’s national transplant information system.

The deployment represents a broader effort to apply decentralized identity and cryptographic verification to public healthcare administration while maintaining institutional oversight and keeping sensitive medical information outside the blockchain.
